PCF Sparkle Care Joins Hands With Eldercare Providers To Enhance Integrated Care Experience For Seniors In Bukit Panjang

PAP Community Foundation (PCF) Sparkle Care has partnered with five eldercare organisations to sign a Memorandum of Understanding (MOU) aimed at creating an integrated and seamless care experience for seniors in Bukit Panjang. Witnessed by Dr Janil Puthucheary, Senior Minister of State for Digital Development and Information & Health, Chairman of PCF Executive Committee, and Mr Liang Eng Hwa, Member of Parliament, Advisor to Bukit Panjang SMC, this signing supports the Ministry of Health’s initiative to streamline the care journey for seniors through an Integrated Community Care Provider (ICCP) framework announced during the Committee of Supply Debate in March. 

Approximately 23,000 seniors currently reside in the Bukit Panjang area, supported by three Active Ageing Centres and three Senior Care Centres, operated by Adventist Nursing & Rehabilitation, Bo Tien Welfare Service Society, Fei Yue Community Services, Precious Active Ageing Centres, and PCF Sparkle Care; with an upcoming Active Ageing Centre by NTUC Health Cooperative Ltd. Currently, seniors and their families may often be required to interact with multiple care providers and undergo repeated assessments. Under the ICCP, seniors requiring various care services – such as Home Therapy, Enhanced Home Personal Care, and Day Care – will benefit from a more coordinated and efficient approach, including: 

  • Seamless Care Coordination: Eliminating the necessity for seniors to navigate multiple service providers 
  • Standardised Care Assessment: Implementing a unified assessment tool across all service touchpoints to ensure consistent evaluation of seniors’ needs 
  • Holistic Care: Facilitating a shared care plan that promotes continuity and quality of care 

 

The implementation of the ICCP in Bukit Panjang will occur in phases, beginning with the collaboration of the Active Ageing Centres to create a consolidated flyer outlining all available activities and programmes for seniors. This resource enables seniors to easily identify and participate in programmes that align with their interests.  

Joint activities will foster social connections among seniors from different eldercare centres, such as active seniors from Precious Active Ageing Centre joining weekly activities with day care seniors at PCF Sparkle Care. Micro-jobs will also be initiated for active seniors to support their peers. Clear roles and workflows will also be established to coordinate seniors’ care needs for timely access to required services. 

Ms Pearlyn Ng, President of Adventist Nursing & Rehabilitation Centre, stated, “This integrated model aspires to foster a seamless and inclusive ecosystem for senior care, simplifying navigation for seniors and their families. Adventist Nursing is committed to collaborating with our fellow eldercare partners to support the seniors in Bukit Panjang.” 

The MOU signing coincided with the official opening of PCF Sparkle Care @ Bukit Panjang. The new senior care centre is designed to provide a warm, home-like environment where seniors can thrive with dignity and maintain independence. As Singapore approaches its super-aged status by 2026, PCF Sparkle Care remains dedicated to strengthening its holistic approach to eldercare. 

Official Opening of PCF Sparkle Care @ Bukit Panjang 

Officially unveiled by Dr Janil and Mr Liang, the centre offers Maintenance Day Care, Dementia Day Care, Active Rehabilitation, and Maintenance Rehabilitation services, with the capacity to serve up to 55 dementia and maintenance day care clients and 30 rehabilitation clients. Beyond being a care facility, the centre will host multi-generational programmes, such as Sparkle Shop and La Kopi. Sparkle Shop and La Kopi are initiatives by PCF Sparkle Care that involve setting up a space that sells snacks or drinks, which aims to create a warm environment for seniors to take part in as shopkeepers and attend to customers from the community. These initiatives serve to foster engagement between seniors and the community, including children and youths, and enhance seniors’ cognitive and social skills. This milestone represents a significant step in PCF Sparkle Care’s ongoing commitment to nearly doubling the number of eldercare centres in Singapore to 25 by 2027. Currently, PCF Sparkle Care operates 13 eldercare centres, with four additional centres scheduled to open between end of 2025 and early 2026. 

Mr Andy Seet, Senior Director of PCF’s Senior Care Division, remarked, “The opening of our new centre provides a comprehensive array of eldercare services tailored to meet the growing needs of seniors and families in the community. PCF Sparkle Care is dedicated to collaborating with other providers to coordinate and enhance support services across the region. Ultimately, we aim to make quality, affordable eldercare accessible for all seniors in Bukit Panjang, enabling them to age gracefully in their own homes and within the familiar surroundings.” 

PCF Sparkle Care @ Bukit Panjang represents an important step forward in Singapore’s eldercare transformation under the national Age Well SG Programme, showcasing the PCF’s commitment to ensuring that every senior has access to holistic, quality eldercare while maintaining connections with their families and communities.
